How to Claim
If you have incurred out of pocket expenses, claims must be submitted to Global Excel Management Inc., who will then coordinate with the provincial plan reimbursement of those approved, eligible expenses. To make a claim, submit to Global Excel Management Inc., the patient name, provincial health plan number and your certificate number with:
- Detailed statements showing the services rendered and the fees charged for each service.
- Mail to: Global Excel Management Inc.

All claims must be submitted to Global Excel Management Inc., within 12 months from the date the eligible services were rendered.
