Continuation of Benefits

In the event of an occupational disability:  All Extended Health Care benefits are continued by the Trust Fund for twelve months, after which you may continue benefits by making Direct Payments.

In the event of a Non-Occupational disability:  All Extended Health Care benefits are continued by the Trust Fund for the first three months of your disability.  Provided you continue to be receiving short or long term disability benefits from the Trust Fund or *Motor Vehicle Insurance Disability benefits, your Prescription Drug benefit will be continued from the 4th to the 60th month of your disability.  You may make Direct Payments to continue your other Health benefits during this period.  Please contact the plan administrator for Direct Payment options.

*Proof of acceptance and monthly benefit payments for your Motor Vehicle Insurance claim must be submitted to the Administrator. It is your responsibility to contact the administrator to confirm your continuing coverage. 

In the event of your death:  Extended Health Care coverage for Dependents shall continue without premium payment following your death up to a maximum of 24 months from the date of death but not beyond the date your Spouse remarries or the date the Dependent is no longer a Dependent, as defined in this website, or to the date the policy or benefit terminates, whichever is earlier.

Any extended benefits payable are subject to the provisions and limitations of the plan.
